What Is “Off-Label” Use?

Sometimes, a licensed medicine is prescribed to treat a health condition that’s not explicitly listed in its official licence (also called a “marketing authorisation”). This is known as off-label prescribing.

It doesn’t mean the medication is unsafe or experimental — in fact, it’s still being used based on solid clinical knowledge and evidence. Off-label prescribing is common in NHS and private practice, especially when there’s strong professional consensus that the treatment is appropriate and beneficial for a specific patient.

Example: A medication originally licensed for type 2 diabetes might also be prescribed for weight loss, based on published evidence, clinical guidelines, and expert experience — even if weight loss isn’t yet listed in the original licence.


How Medicine Licensing Works (UK & EU)

Before a medicine is licensed for any use, it must go through multiple phases of clinical trials and rigorous safety reviews by official bodies like:

  • MHRA – Medicines & Healthcare products Regulatory Agency (UK)
  • EMA – European Medicines Agency (EU)

If successful, a marketing authorisation is issued, confirming the medicine can be used for specific conditions. The licence includes:

  1. Approved uses and patient groups
  2. Safe dosage and usage instructions
  3. Side effects, warnings, and interactions
  4. Storage and handling guidance

All this information is outlined in the Patient Information Leaflet (PIL) provided with every medicine.


Why Off-Label Use Happens

Off-label prescribing is guided by clinical judgement, patient safety, and often supported by:

  • National or international clinical guidelines (e.g. NICE, ADA, Endocrine Society)
  • Published research studies and peer-reviewed journals
  • Experience from frontline practice (NHS, private, hospital)

Prescribers may consider off-label use when:

  • A treatment is proven effective in practice, but not yet officially licensed for a newer use
  • The licensed alternatives are unsuitable or unavailable
  • There’s strong safety and monitoring infrastructure in place
